Crashed into power lines on approach to Prospect Bay, Rainy Lake, near Fort Frances, Ontario 5 October 2004. Pilot escaped and the aircraft sunk in shallow water. The tail assembly was noted at Red Lake seaplane base during the same trip. A visit in June 2017 found that the yard has now been cleared of wrecks.
