XX164 crashed on take-off at RAF Valley on 13 Feb 1996 and was destroyed. Sadly, the pilot was killed. The cause was a disconnected aileron control rod that caused an uncommanded and uncontrollable roll immediately the aircraft left the ground. RHB.
